It can now be filled in on-line, you will need to print it and have it signed and email back to marcadmin@marcatlanta.org , or mail to MARC, P O Box 467292, Atlanta, GA 31146. Please keep a copy of the completed form for your records. Payments can be made via credit card online or check attached to the application.
